Teeth is dedicated to protecting your privacy rights and confidential information. Our commitment is to ensure that your health information is never compromised. Teeth may, from time to time, amend this privacy policy but will always inform you of any changes that affect you or your care.
Protecting your personal healthcare information: Teeth may disclose the information that we collect from you only as allowed by the Health Insurance Portability and Accountability Act and the State of Colorado. This includes issues relating to your treatment, payment, and our health care operations. Our electronic system is protected from any unauthorized users. We ensure that the confidentiality of your records is protected, whether you are a current or past patient. Your health information will not be improperly disclosed or released. Your personal health information will never otherwise be given to anyone, even family members, without your written consent. You may provide Teeth with written authorization for us to disclose your information to anyone that you choose. Please ask me about my HIPAA form.
Collecting your data: Teeth will only request personal information that is pertinent to your care, such as your name, address, phone number, medical history, health records, employment data, etc. If you fill out my booking form through the website, it will ask you to provide your first and last name, email address, and phone number. This information will be used to contact you in order to set up a potential future appointment. Teeth will also gather information in order to collect payments for services, conduct normal health care practices, and in order to comply with the law.
Disclosing your information: In certain instances, Teeth may have to disclose information as required by the law. Teeth is obligated to provide information to law enforcement and government officials, should they ever request it. Teeth will also provide your diagnostic information to a dentist or specialist with whom we agree may be beneficial to your care. This will be discussed and approved by you before the information is shared with any other healthcare provider. We may disclose your appointment information via text or voicemail to the number your provided in order to confirm your upcoming appointment.
Patient rights: You have a right to request a copy of your health records at any time. If you ever wish to amend your HIPAA consent form or approved contacts, please let us know and we will give you a new form. If you believe your privacy rights have been violated, please notify Teeth right away, or notify the U.S. Department of Health and Human Services.

Teeth is hosted on the Wix.com platform. Wix.com provides Teeth with the online platform that allows this business to market it's services to you, and collect your personal information (name, phone number, email) via the voluntary booking form. Your data may be stored through Wix.com’s data storage, databases and the general Wix.com applications. They store your data on secure servers behind a firewall.
